1.Install from WP Repo, or unzip package and upload to the plugins directory and activate

- Note: this plugin applies only minimal styling to form and content.
- The modal can be activated on page load or click.
- If the plugin is set to show on page load, when closed it will store the session in the browser and should be visible only once per visit.
